If you have any insurance other than Cigna or Aetna, please visit this link or QR code to determine insurance eligibility with The Lactation Network (TLN).  If covered, these visits are at no cost to you! It is important to fill out the TLN form and receive approval before your first visit. 

*Cigna and Aetna Clients: this step is not necessary.  Grounded Lactation is directly in network with these companies. Please request an appointment below.  

*Duke Select Aetna clients: Please request appt below and you will be sent pre-authorization information. There are no lactation consultant practices in Grounded Lactation's practice area that are directly in network with Duke Select AND that don't charge additional fee for home visits.  Grounded Lactation has 100% approval thus far with Duke Select pre-authorizations and doesn't charge home visit fee (unless outside 15 mile travel radius).

Clients under the The Lactation Network umbrella (BCBS, Anthem, United, etc): 

The Lactation Network will submit all claims directly to your insurance company on behalf of Grounded Lactation.  The Lactation Network will appeal all cost-sharing under the Affordable Care Act which states that lactation services are preventive and not subject to cost-sharing. 

Aetna and Cigna clients: 

Grounded Lactation will submit all claims to your insurance company on your behalf.  If visit quota has been met, Aetna may apply cost-sharing to you, with your bill being the difference between my self-pay rates and what insurance re-imbursed.  Cigna generally does not apply cost-sharing for any visits (even beyond 6 visits).

Self-pay (out-of-network) clients: $200 initial; $150 follow-up; sliding scale-payment determined by client.

For out of network visits, you will be provided with a superbill with all the information you need to seek reimbursement from your insurance company and will serve as a payment receipt. You agree to pay Grounded Lactation at the time of the visit (cash, check, credit card, FSA).  

Cancellation Fee: $100

Any appointment not cancelled within 12 hours of appointment time. Notice via email is sufficient. 

Outside Home Visit Radius: $25/visit

Any home visit outside of my 15 mile home radius is subject to a $25/visit charge. This is required at time of visit and is not covered by insurance. Please refer to groundedlactation.com homepage for radius map.
